is that i am required to do the medical exam in the same site as i did in the first time? because i may be required to do medical exam again as my expiration date is coming near. am i allowed to take the second exam in other places or other countries?

Yes you can.
But you have to inform the CIC office you applied for so they can send you the list doctors at your new location.

you can do it wherever you want as long as its done with a DMP, could you share your timeline and which office your applying through?
