is the form fd-961 the form for fingerprinting i went to fbi website and found only this form for fingerprinting can anyone advise is this the right form to use for fingerprinting to obtain police clearance for canadian immigrati

you don´t need any form for canada PCC and fingerprinting.
just go there with two piece of ID they will do it everything for u.
