Subject: URGENT!! PLEASE ANSWER!!
  Does both contract and reference letter need to have the following?

? the specific period of your employment with the company
? the positions you have held during the period of employment and the time spent in each position
? your main responsibilities in each position
? your total annual salary plus benefits
? the signature of your immediate supervisor or the personnel officer of the company
? a business card of the person signing

On my employment contract which has specified my position, salary and responsibilities but not on the reference letter as it major mentioned my performance at work.

So is that going to be ok? Or both contract and reference letter must specify them?

As long as either the contract or reference letter has the above mentioned information you should be fine.

So as long as my contract has mentioned all, the reference letter doesn´t have to mention them again, right?

I think your designation; period and contact details are fine. I submit 4 employment letters with designation, duration and contact details it worked fine.

I asked my employer to make me two letters...

One saying the no. of hours I worked with them... I was working on and off with them...

Second, stating my duties and responsibilities at the job. I made them write my salary at the time I got the letters...

I was doing a part time job for almost 3.5 to 4 years...
