Address on medical forms

Subject: Address on medical forms
My medical forms do not show the new address because I moved. CIC has updated their records and on the web. Is it okay if I hand write the new address on the medical forms before doing the medicals?

Hi Thu...sorry I cannot answer your query but maybe I can share my case: In my daughter´s medical form, her name is misspelled...my lawyer said that the doctor can change that....maybe you can also ask your doctor to fill that up for you.

If you don´t mind, I have a question for you:

The front page of my medical instruction mentioned something about....look for important instructions at the back of this page.....

The problem is the form that they sent me has no back page. Does your instruction have back page? Is it too long? or can you list down the instructions here?

I assume you are talking about the form letter medical\inst_sbj/Feb04 addressed to you. this does have a back page with some instructions; most of which are obvious such as 2 photographs, proof of identity, x-ray detail, period of validity. Also take eye glasses and list of prescription drugs or any relevent medical reports, ask doctor to answer all questions in sections B through E attach original lab reports and forward original chest x-rays the address in Ottawa.
