I need some information regarding the completion of the form Access to Information Request Form.
It is about the sponsorship of my wife in China.
Is the name of the applicant on this form my wife´s name or my name? (Because I applied for sponsorship).
I suppose the signature must be my wife´s, as the notes will be concerning her file in Beijing. Am I right?
Your help will be greatly appreciated.

your wife must give permission for you to see her file... even though you are the sponsor. Her name goes in the top, your information goes in the lower section, and she must sign the document.
