I´ve looked around for this form to add to my husband´s immigration, but can´t seem to find it. I ended a common law relationship in Oct. 2004, but never declared it as severed. HELP!

Hi I just found out that I need this form. I never severed the common law relationship which ended in Dec 2006. I´ve since married someone else. Where do I go in Toronto to get this done (cheaply)
