I have a couple of questions, I would really appreciate if you can help me and also this would be helpful for the people who are applying for CAIPS..

Is that the request form
In the form,
2) Federal Govt. Institution
Is it CITIZENSHIP AND IMMIGRATION
3) Provide details regarding the information being sought:
Is it just "CAIPS notes for my PR application"
4) Method of access preferred
Is it receive copies of originals or the other one
5) I am sending my bank statement for address proof
here I am worried because when I sent ROLF to buffalo, I had 10,000 dollars, but now, I have only 780 dollars now. So if I send this, will it create any problems that I don´t have required amount in my bank statement?

yes, yes, yes, yes, any official documents with your name and canadian address, plus add in your current visa copy.

........I changed my address last month, only document that I have for the address proof is my bank statement. But it shows only remaining 600 dollars in that, will that create a problem that I have less than the minimum amount (10,000 dollars) ?? I had to send the bank statement with ROLF just in June.

I don´t think there will be any problems with bank statement. One can have more than one bank account anyway. :)
