Hey Isabella,
I completed everything for CAIPS and about to post, one of my friends confused me that the form I am using is wrong. Which one did you use,

I have seen many using both the forms.
I have used the second one.

If I would be you, I will be using second one, Because it has all the information they need to have to process your request and its from CIC.
If you notice the date of revision of both the forms. Second one is latest, i.e 10-2004 v 2000/06/19.
Use second form.

3) I am in Canada so I am not checking ACCESS TO INFORMATION ACT
4) I am checking PRIVACY ACT
5) I am checking "RECEIVE COPIES OF THE RECORDS"
6) In REQUEST FOR, I am checking only CAIPS and entering the information. I am not checking anything else in this.
That would be really amazing if you can just answer with an YES or NO for the above questions. It is not only very useful for me but also for people who are new for CAIPS, I really appreciate for sparing some time.
