What is this "medical expire" issue? i) How it expires and ii) how to be careful to prevent the expiration?
Any suggestions?

1) Medical results are valid for 1 year. So if the immigration process is not complete within one year of the medical exam the applicant MAY have to get the medical done again.
2) Get the medicals done only when instructed to do so.

if your medicals are due to expire soon , you can apply for an extension, most visa offices will accept that. you can do nothing to avoid this situation because after medicals you can´t tell when your visa will be ready.
